What year and model ford rotors do I need to convert to disc

You might check on the Commando BBS. They have had a bunch of guys talking about disc conversions. The hot ticket seems to be the 78' CJ rotors, with the chevy backing plates and calipers. Then you need to find an extra 1/8 inch to space your hub out so everything lines up nice. A spacer, or a little machining on the hub, or grinding an 1/8 inch off of the brake pad to make things line up. This applies to closed knuckle, and I think that it will work for the open knuckle 30's too. Check out this link on a CJ2 disc conversion. It is one of the best I have seen.
