got codes from the check engine light, now what?

Alright well my Jeep has been running like crap. And the CEL always comes on after driving for a minute. I checked the codes and I got a:
--12* Direct battery input to PCM was disconnected within the last 50 key-on cycles
--33* An open or shorted condition detected in the A/C clutch relay circuit.
--21** Neither rich nor lean condition detected from the oxygen sensor input.orOxygen sensor input voltage maintained above the normal operating range.
--52** Oxygen sensor signal input indicates rich air/fuel ratio condition during engine operation.

So hopefully that tells me whats wrong with it, but now how can I fix that? Do I need a new 02 sensor? should I get a new fuel filter? just looking for some help here. also, the engine has almost 110,000 miles on it, should I be replacing stuff?

Re: got codes from the check engine light, now what?

The first two 12 and 33 wouldn't affect the way it runs, but the 21 and 52 indicate that either the O2 sensor, or it's wiring is bad.
Generally, your supposed to replace O2 sensors after 75000 miles or so, and with 110k, chances are it's starting to fail. I would start by changing it.
