Not that they're maintenance intensive but they are diesels and they require a bit different maintenance than a gasser. A lot of folks treated them like small blocks and basically just changed the oil and otherwise neglected them. You can't do that with any diesel.
Others also figured, "it's a diesel, it must be a dump truck motor" and then worked them to death. They were only meant to do what someone might do with a 350 except get much better gas mileage.
